About Me

Rachel Rafeq, PharmD is an Emergency Medicine Clinical Pharmacy Specialist and the Residency Program Director of the PGY2 Emergency Medicine Program.  She is the founder of Emergency Medicine Pharmacy Services at Cooper Hospital and the PGY2 Emergency Medicine Residency.

Dr. Rafeq earned her Doctorate of Pharmacy degree at the Massachusetts College of Pharmacy and completed two years of post-graduate residency training at Umass Memorial Hospital, with a second year in Medication Use Safety and Informatics. However, due to her clinical interest in emergency medicine she continued to pursue this path further.

Dr. Rafeq is a contributor and editor on criticalcarenow.com and the creator of the Instagram account @pharmbythegram. Her clinical interests include toxicology and addiction medicine.

About Me

David Fett, Pharm.D., BCPS is an Emergency Medicine Clinical Pharmacy Specialist and Program Coordinator for the PGY2 Emergency Medicine Pharmacy Residency at Cooper University Hospital in Camden, New Jersey.

Dr. Fett earned his Doctor of Pharmacy degree from the Ernest Mario School of Pharmacy at Rutgers University in New Brunswick, NJ. As post-graduate training, he completed a PGY1 pharmacy practice and PGY2 Emergency Medicine Residency at Boston Medical Center.

Dr. Fett’s research interests include anticoagulation, addiction medicine, and pharmacy education. In his free time, Dr. Fett enjoys the outdoors and playing with his corgi.

About Me

Terence Chau, PharmD, BCCCP, completed his PGY1 Pharmacy Practice residency at Henry Ford Hospital in Detroit, Michigan. He completed his PGY2 Critical Care Pharmacy residency at Memorial Hermann – Texas Medical Center in Houston, Texas. Upon graduation in 2011, he joined Memorial Hermann Memorial City as the emergency medicine clinical pharmacy specialist; and served as the PGY1 Pharmacy Practice Residency Program Director from 2017 to 2020. He recently transitioned to a critical care clinical pharmacy specialist position at Cooper University Hospital. Research interest includes sepsis, coagulation, and resuscitation.

About Me

Annmarie Cristiano, PharmD, DPLA is the Medication Safety Clinical Pharmacy Specialist at Cooper University Health Care. She received her Doctor of Pharmacy degree from the Philadelphia College of Pharmacy in Philadelphia, PA. She completed a PGY1 Pharmacy Practice Residency at MedStar Washington Hospital Center in Washington D.C., and a PGY2 Health System Pharmacy Administration and Leadership (HSPAL) Residency at MedStar Health in Columbia, MD. At CUHC, she chairs the Medication Safety Committee and co-chairs the Anticoagulation Committee.

Dr. Cristiano's clinical interests include smart pump technology, wellness and burnout, operating room (OR) pharmacy services, and leadership development.

Justin Delic, PharmD, BCCCP is a Clinical Pharmacy Specialist in Critical Care at Cooper University Hospital.  He earned his PharmD from the Duquesne University Mylan School of Pharmacy in 2014 and then went on to complete a PGY1 Pharmacy Practice Residency at University of Pittsburgh Medical Center Mercy Hospital in Pittsburgh, PA, in 2015 and a PGY2 Critical Care Specialty Residency at University Hospitals Case Medical Center in Cleveland, OH, in 2016.  Prior to his current role, Justin was an Assistant Professor of Clinical Pharmacy at the Philadelphia College of Pharmacy.  His research interests include nutrition support, neurology, advanced cardiovascular life support, and educational technology.

Lauren A. Igneri, PharmD, BCPS, BCCCP, is a Clinical Pharmacy Specialist in Critical Care. Dr. Igneri earned her Doctor of Pharmacy from the Ernest Mario School of Pharmacy at Rutgers University and completed a PGY1 Pharmacy Practice Residency at Temple University Hospital followed by a PGY2 Critical Care Residency at University of the Sciences in Philadelphia and CUH.

Dr. Igneri is an active member of the Society of Critical Care Medicine and contributes regularly within the Clinical Pharmacy and Pharmacology Communications Committee. In 2019, she was elected Member at Large of the Women in Critical Care Knowledge Education Group. Additionally, Dr. Igneri is a member of the American College of Clinical Pharmacy and the American Society of Health-System Pharmacists. She has served as faculty creating educational materials for ASHP Board Certification Resources since 2016.

After graduation, she completed a pharmacy practice residency at Atlantic Health System followed by a Pediatric Pharmacotherapy residency at Rutgers University. About Me

Diana Solomon, PharmD, BCCCP, is a Critical Care Clinical Pharmacy Specialist. Her primary clinical service coverage is in the medical/surgical ICU; she rotates through Trauma and Neuro critical care services as well.

Dr. Solomon earned her Doctor of Pharmacy degree from the Philadelphia College of Pharmacy at the University of the Sciences in Philadelphia, PA. As pot-graduate training, she completed a pharmacy practice residency at Shore Medical Center in Somers Point, NJ and then specialized in critical care at the Philadelphia College of Pharmacy in conjunction with Cooper University Hospital.

Dr. Solomon’s research interests include pharmacotherapy and pharmacokinetics in critical illness and extracorporeal circuits, nutrition support therapy, and education. In her free time, Dr. Solomon enjoys studying, practicing, and teaching yoga.

About Me

Madeline King, PharmD, BCIDP, completed her PGY1 pharmacy practice residency at NewYork Presbyterian Hospital in New York. She completed her PGY2 infectious diseases residency at Temple University School of Pharmacy. Upon completion of residency, she took on the role of assistant professor of pharmacy at the Philadelphia College of Pharmacy where she taught infectious diseases and led a global public health rotation in Guatemala. She was the PGY2 infectious diseases residency director for the 2021-22 residency year. In 2022 she joined the team at Cooper University Hospital as an outpatient antimicrobial stewardship pharmacist. In this role she is working to improve outpatient antibiotic prescribing within the Cooper healthcare system, as well as transitions of care for patients on OPAT.

Kristi (Soyoung) Kim, PharmD, BCCCP, is a Clinical Pharmacy Specialist in Critical Care at Cooper University Hospital. She earned her Doctor of Pharmacy degree from the University of Texas at Austin School of Pharmacy and completed a PGY1 Pharmacy Practice Residency at Memorial Hermann Memorial City, followed by a PGY2 Critical Care Residency at CHI St. Luke’s Baylor Medical Center in Houston, Texas. Dr. Kim is an active member of the Society of Critical Care Medicine and participates in multiple committees within the Clinical Pharmacy and Pharmacology Section. Her clinical interests include analgesia and sedation in the critically ill, anticoagulation, and medication safety.

Akhilesh Sivakumar, PharmD, BCOP is a Clinical Pharmacy Specialist in Oncology at Cooper University Hospital/MD Anderson Cancer Center at Cooper. He earned his PharmD from The Ohio State University College of Pharmacy in 2019 and went on to complete a PGY1 Pharmacy Practice Residency at Emory University Hospital in 2020 and a PGY2 Oncology Pharmacy Residency at Emory University Hospital Midtown/Winship Cancer Institute in 2021, both in Atlanta, GA. His areas of interest include hematologic malignancies, especially multiple myeloma. Outside of pharmacy, he enjoys playing and watching tennis, playing video games, and spending time with his family and friends.

Geena Kludjian, PharmD, BCIDP is an Infectious Diseases/Antimicrobial Stewardship Clinical Pharmacy Specialist at Cooper University Hospital. She earned her PharmD from Philadelphia College of Pharmacy in 2019 and then went on to complete a PGY1 Pharmacy Practice Residency at Temple University Hospital in Philadelphia, PA, and a PGY2 Infectious Diseases Pharmacy Residency at Temple University School of Pharmacy. In addition to her current role, she is also an Assistant Professor of Medicine at Cooper Medical School of Rowan University. Her main interests include teaching, antiretroviral stewardship, management of patients with antibiotic allergies, and management of gram negative bacteremia.
